To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by:

  • Free / Reduced Lunch requirements : In order for a qualifying student to receive a free or reduced price Type A meal, they must choose a 1/2 cup serving of fruit or vegetables with their meal.  Otherwise, students will be charged a la carte pricing.
