There will be a Mobile Dental Clinic available for Williams Valley students on September 4, 5, and 6. This service is completely optional and includes exams, cleanings, fluoride, x-rays, sealants, and fillings (if needed). If you would like your child to participate, please complete the form at the appropriate link below by August 27. Paper forms will be sent home with students on the first day of school. Please be aware that junior/senior high school students will be transported by van to the elementary building where the dental team will be set up.
As indicated on the form, there is no cost if your child is insured by Medicaid or PA CHIP. Please read the entire form for complete information regarding charges. All students who are seen by the mobile dentist will get a dental report that will list the services received.
